It's the exact same with a sword; it doesn't just turn into a club. The pommel doesn't do as much balancing as you think either. Most of it comes from the blade's taper.

Although there are numerous strikes with the pommel while the sword is sheathed and occasionally with both hands on the hilt, I've never seen somebody completely reverse the sword and grip the blade with both hands. The maneuverability lost is negligible, and it protects your hand better. This graphic is obviously intended to be funny, but you absolutely can hold a sword like that (and probably should!).
