Except as provided in 123.3 and 123.5, mailers must present at the time of mailing a fully completed sender’s declaration (the Post Office™ copy of PS Form 2976 or 2976-A), which specifies both the sender’s name and address and the addressee’s name and address.

The revised PS Form 2976 has an edition date of September 2012 and includes several enhancements to the December 2009 edition. Images of various pages of the revised form appear on pages 10 – 11 in this issue of the Postal Bulletin.

Beginning August 28, 2016, we will use PS Form 2976-R to replace the following hard copy customs declaration forms: PS Form 2976, Customs Declaration CN 22 — Sender’s Declaration; PS Form 2976-A, Customs Declaration and Dispatch Note — CP 72; and PS Form 2976-B, Priority Mail Express International Shipping Label and Customs Form.

Handwritten Customs Forms (PS Form 2976, 2976-A, or 2976-B) After March 13, 2020, inform any customer that presents a mailpiece with a handwritten form that USPS regulation changes now require electronic submission of customs data.

Electronically generated customs forms PS Form 2976, PS Form 2976–A, and PS Form 2976-B produced through an approved USPS system capable of electronically transmitting the associated customs data.
