Insulin pen devices have shown comparable efficacy to the traditional vial/syringe method, and some evidence exists to suggest that this efficacy is also similar and potentially better in the elderly population. Additionally, the perception of efficacy has been shown to be higher in patients using pen devices. Ease of Use

The essential steps in starting insulin, whether by syringe or by pen, are outlined in Box 2. Elderly patients receiving insulin should monitor capillary blood glucose; the effect of ageing on the renal threshold for glucose means that urine testing is of little value for assessing glycaemic control.

Premixed insulin analog therapy may provide added convenience, as well as improved glycemic control. Insulin pen devices are also recommended to facilitate insulin dosing and help patients maintain their independence.

The presence of comorbid conditions may interfere with older adults' ability to inject insulin and monitor their blood sugar appropriately. Specialized devices, such as insulin pens, may offer a means to assist older adults with their insulin therapy.
