In a mature cortical cataract, also known as a "white cataract," the entire lens cortex has become opacified. Water clefts are visible as less opaque radial wedges within the lens. These lenses can swell as a result of increased water uptake, at which point they are called intumescent cataracts.

A cortical cataract develops in the outer layers of the cataract (“the cortex”) and grows slowly in a spoke-like fashion from the lens periphery to the center. These “spokes” can cause light rays entering the eye to scatter, which in turn causes blurry vision, glare, haloes and issues with contrast sensitivity (how sharp an image seems).
