The 4AT is designed to be used as a delirium detection tool in general clinical settings, inpatient hospital settings outside of the Intensive Care Unit (ICU), or in the community. The 4AT is intended to be used by healthcare practitioners without the need for special training, and it takes around two minutes to complete. [ 4 ]

For the early detection of delirium, a feasible and valid screening tool for healthcare professionals is needed. This study aimed to present a scientific reason for using the 4 ‘A’s Test (4AT) through a systematic review and meta-analysis of studies on the diagnostic test accuracy.

The 4 A’s test (4AT) is a short screening tool for delirium and cognitive impairment developed in the UK . It has been translated into several languages, and validation studies report high sensitivity and specificity for the diagnosis of delirium [11–18].

2024年12月4日 · The 4AT is suitable for use in normal clinical practice and allows assessment of severely drowsy or agitated patients. Due to practice effects with specific test items, the 4AT is not designed for repeated daily monitoring for delirium. The 4AT can be used at first patient contact or at other times when delirium is suspected.

There is a need for a rapid delirium screening tool that can be administered by a range of professional-level healthcare staff to patients with sensory or functional impairments in a busy clinical environment, which also incorporates general cognitive assessment. We developed the 4 ’A’s Test (4AT) for this purpose.

The 4AT is a short delirium detection tool that can be used to identify delirium in terminally ill people receiving palliative care in hospice inpatient settings. Embedding the 4AT within standard clinical assessment is recommended.
