Amyloidosis (AL) is known for a highly variable clinical presentation. Amyloid deposition leads to hepatomegaly (50%), splenomegaly (10%), peripheral neuropathy, renal amyloidosis, and carpal tunnel syndrome (25%). Cardiac involvement causes conduction defects, arrhythmias, congestive cardiac failure and may account for 40% of deaths .

This is the first report of localized AL amyloidosis of the mobile tongue that is suggested to be associated with median rhomboid glossitis. In addition, chronic irritation of the lesion might be associated with etiology of chondroid metaplasia through the upregulation of SOX9 expression.

2022年12月1日 · Based on this review, we propose a diagnostic algorithm for macroglossia according to the characteristics described. The most common diagnoses among acquired causes were amyloidosis (13.7%), endocrinopathies (8.8%), myopathies (4%) and tongue tumors (6.7%).

The results of an incisional biopsy showed an amyloid on Congo red staining and positive findings for the κ light chain by immunohistochemical analysis. The findings of the systemic workup were negative.

We report a 73-year-old female who presented with weight loss and macroglossia. Firstly, she was diagnosed only with amyloidosis of the tongue. Her general health condition was evaluated, revealing renal dysfunction, anemia, hypercalcemia, and hyperphosphatemia. The final diagnosis was systemic amyloidosis with multiple myeloma.

Amyloid is usually deposited in the tongue, as a part of generalized amyloidosis. Isolated localized amyloidosis of the tongue is relatively rare. We report such a case which presented with glossodynia and glossopyrosis and was thought clinically to represent median rhomboid glossitis.

A case of median rhomboid glossitis associated with amyloid deposition was presented. Clinically there was no organ involved in amyloid other than the tongue. It was suspected that median rhomboid glossitis occurred first and that amyloid was induced later.
