A single course of betamethasone is recommended for pregnant women between 34 0/7 weeks and 36 6/7 weeks of gestation at risk of preterm birth within 7 days, and who have not received a previous course of antenatal corticosteroids.

Current international recommendations are to initiate treatment with betamethasone (12 mg every 24 h, 2 doses) or dexamethasone (6 mg every 12 h, 4 doses) [5], in any gestation at risk of preterm delivery from 24 + 0 weeks gestational age (GA) to 34 + 0 weeks GA [6, 7].

When used for preterm labor, betamethasone is given as an intramuscular injection (a shot). [3] ACOG recommends two 12-mg doses given 24 hours apart. Regularly scheduled repeat courses of the medication are not currently recommended, and medication should be reserved for cases when a pregnant person is at risk of preterm delivery within seven days.

In preterm premature rupture of membranes at less than 32 weeks' gestation in the absence of clinical chorioamnionitis, antenatal corticosteroid use is recommended. Treatment at 32 to 33 weeks may also be beneficial. Patients eligible for therapy with tocolytics should also be eligible for treatment with antenatal corticosteroids.

Betamimetics help to delay delivery for women transferred to tertiary care or completed a course of antenatal corticosteroids, but are not recommended as a first line tocolytic or for long term therapy.
