Displaced femoral neck fractures (FNFs) in healthy elderly patients have traditionally been managed with hemiarthroplasty (HA) or total hip arthroplasty (THA), with studies suggesting that THA may be the better option.

Unipolar and bipolar hemiarthroplasty were compared with THA within patient subgroups defined by age (60 to 79 versus ≥80 years) and American Society of Anesthesiologists (ASA) classification (I or II versus III); patients with an ASA classification of IV or higher were excluded.

We aimed to clarify the possibility of using bipolar hip arthroplasty (BHA) for degenerative diseases by examining long-term results postoperatively in 336 hips. Patients’ average age was 61.0 years (range, 34–88 years), and the average follow-up period was 12.5 years (range, 5.0–27.3 years).

Many treatment methods such as proximal femoral nail (PFN), dynamic hip screw (DHS), external fixator, unipolar and bipolar hemiarthroplasty (BPH) have been used in the treatment of these fractures [6,7]. However, it is very difficult to provide stable fixation in these unstable class fractures due to decreased bone quality.
