A Voluntary Declaration of Parentage (or paternity) (a VDOP) is a government form two parents can sign to create a legal parent-child relationship between a child and parent. It's often signed at the hospital when a child is born, but it can be signed later.

The Parentage Opportunity Program offers eligible parents the opportunity to establish a legal parent-child relationship free of charge by signing and filing a Voluntary Declaration of Parentage.

The attached Voluntary Declaration of Parentage (VDOP) form is to be used by parents who are voluntarily agreeing to establish parentage. Completing and signing the form is voluntary.

What is the Parentage Opportunity Program? The Parentage Opportunity Program (POP) administers the Voluntary Declaration of Parentage process so a birth parent and other parent can establish legal parentage of their child free of charge.

To receive the Parentage Opportunity Program Online Access Request form, please email [email protected] and request the form. The intended user’s supervisor must submit a completed form to program staff for processing.

2020年1月1日 · To ask a judge to cancel (set aside) a Voluntary Declaration of Parentage or Paternity (VDOP), you fill out and file a court form. When you file the form, the clerk will give you a court date (a hearing date). At the hearing, you and the child's other parent will both get the chance to speak to the judge. Then, the judge will make a decision.

There are two ways to cancel a Voluntary Declaration of Parentage (or Paternity) (VDOP). Canceling (or rescinding) the VDOP ends the legal parent-child relationship created by the VDOP.

You are legally the parents of this child when you sign and file this Voluntary Declaration of Parentage (VDOP). You have the right to speak to a lawyer or to request genetic testing before you sign. You must be eligible to sign or it will be legally invalid.
