Dry needling (DN) therapy uses a thin filiform needle to penetrate the skin and stimulate underlying myofascial trigger points (MTrPs) and muscular, and connective tissues. Dry needling is defined by Western medicine as a type of acupuncture [1].

In the treatment of scar tissue with dry needling, the needle’s rotation stimulates fibroblast activity, promoting collagen release and faster tissue restoration; that is, winding the connective tissue around the inserted needle leads to the further displacement of the surrounding cellular matrix.

Results: Two randomized controlled trials found that dry needling of scars (using the classic "surrounding the dragon" technique in one study and intradermal needling in a second study) was more effective than sham or control interventions with respect to scar appearance and pain.

Here is my general thought process on needling scars: If I can do it safely, I always want to treat the scar until I can get a needle directly through it, down to the periosteum. If the scar is over the abdomen, lung field, or something else you don’t want to hit, needle shallowly into the scar tissue.
