ECG changes in left ventricular hypertrophy (LVH) and right ventricular hypertrophy (RVH). The electrical vector of the left ventricle is enhanced in LVH, which results in large R-waves in left-sided leads (V5, V6, aVL and I) and deep S-waves in right-sided chest leads (V1, V2).

These are normal in left-sided chest leads (V5, 6, lead I, aVL) as they represent septal depolarization from left to right. This is as long as they are <0.04secs long (1 small square) and <2mm deep.

Lead V5 detects a very large vector heading towards it and therefore displays a large R-wave. Lead V1 records the opposite and therefore displays a large negative wave called S-wave.

ST segment depressions are seen in leads V5, V6, aVL and I. The hallmark of left bundle branch block is the wide QRS complex (QRS duration ≥0.12 s), deep S-wave in V1–V2, large and clumsy R-wave in V5, V6, aVL and I. These ECG changes are shown in Figure 7, which should be studied carefully.
