2024年12月19日 · De Quervain tenosynovitis, also known as washerwoman's sprain/strain, is a painful stenosing tenosynovitis involving the first extensor (dorsal) tendon compartment of the wrist (typically at the radial styloid). This compartment contains the abductor pollicis longus (APL) and extensor pollicis brevis (EPB) tendons.

The APL measurements for thickness and width were performed at their point of insertion. The EPB and EPL were measured for thickness and width at their intersection point with a line perpendicular to the first metacarpal insertion of the APL (Figure 1).

HOW DOES DE QUERVAIN'S TENOSYNOVITIS HAPPEN? The EPL and APB tendons run alongside each other, and have closely related functions: abduction (make a "C" or an "O" of your thumb and fingers). These tendons run through a synovial sheath (imagine it to be like a blanket that wraps around the tendons) which contains them.

APL: abductor pollicis longus; APB: abductor pollicis brevis; EPB: extensor pollicis brevis; EPL: extensor pollicis longus. In specimens 9 and 10, the tendon of the EPL, which is usually single, was found to be double and located in the third extensor compartment.

We will describe a case of an asymptomatic accessory EPL that was found incidentally during surgery with a tendon located in the fourth extensor compartment.
