Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumors of the gastrointestinal tract. They account for ~5% of all sarcomas and are mostly found within the stomach and mid-distal small bowel. They respond remarkably well to chemotherapy.

The standard treatment of localized, resectable and non-metastatic GISTs and EGISTs is surgery, with combination of imatinib and surgery when high-risk, recurrence or metastasis is present [7, 8].

On magnetic resonance imaging (MRI), GISTs typically show low signal intensity on T1-weighted imaging (T1WI), high signal intensity on T2WI, and enhanced signal intensity on post gadolinium images [33].

Small GISTs had MRI features different from the well-known imaging features of large GISTs. The presence of intratumoral cystic change and a low mean ADC value may be helpful for predicting the high malignancy potential of GISTs.
