The Uniform Application for Licensure (UA) is a web-based application designed to enhance the nation’s state-based licensure system. The UA improves license portability by eliminating the need for physicians, physician assistants/associates (PAs) and resident applicants to re-enter information when applying for licenses in multiple states.
FSMB | UA Fees and Process
Applicants using FCVS for Requiring State Medical Boards will receive the Uniform Application at no extra charge. The Uniform Application has a onetime non-refundable fee of $60 for the initial application of each MD, DO or PA license for Non-Requiring State Medical Boards for applicants who have not completed a FCVS profile.
