When blood vessels get inflamed, they can bleed into the skin, causing a reddish-purple rash (purpura). The most striking feature of this form of vasculitis is a purplish rash, typically on the lower legs and buttocks.

Purpura not due to a low platelet count, caused by inflammation in blood vessels of the skin, is the hallmark of HSP. The tetrad of purpura, arthritis, kidney inflammation, and abdominal pain is often observed. However, all four elements of this tetrad are not required for diagnosis.

HSP is the most common vasculitis of childhood. It most commonly affects children 2-8 years of age. Usually affects large joints of lower limbs. Rarely upper limbs. If prescribing oral prednisolone, give 1-2 mg/kg/day (maximum 60 mg/day) or IV methylprednisolone 0.8-1.6 mg/kg/day (maximum 1 g/day) while symptoms persist.
