Horizon Adult Health Care provides healthcare in a home-like setting for adults in 14 counties in southern and southeastern Kentucky.

Horizon Adult Health Care Centers care for those needing occasional or temporary care, as well as candidates for a nursing home. It is difficult for me to get my loved one in and out of the car. Is transportation available? Yes! Horizon Adult Health Care operates a fleet of several vans that have been modified for those with disabilities.

For 20 years, Horizon Adult Health has cared for senior loved ones at our 11 centers in Kentucky. We are now providing that same loving, reliable care in the home. All of our services will help make sure your loved one is secure, healthy and connected to the community, which is important for mental well-being.
