Individualized Family Service Plan (IFSP) Under Part C of IDEA, the IFSP is required to enhance the capacity of families to meet the needs of children birth to age three who have developmental delays or disabilities.

Form 4207 constitutes the required final pages of the Individualized Family Service Plan (IFSP) used to record the Early Childhood Intervention (ECI) services planned for the child. Page 1 is also used to fax information to the Health Passport for children in foster care.
The Individualized Family Service Plan (IFSP) is a written plan that is developed for each eligible infant and toddler with a disability. The Part C regulations specify, at 34 CFR §§303.342 - 303.345, the procedures that State Lead Agencies and early intervention service providers must follow to develop, review, and revise an IFSP for each child.
