Endoscopic ultrasound (EUS) is a minimally invasive endoscopic technique which allows the doctor (endoscopist) to obtain detailed images of the pancreas. EUS provides the endoscopist with more information than that obtained with CT or MRI imaging. EUS can be used to evaluate solid masses and to evaluate cystic masses of the pancreas.

When only intraductal papillary mucinous neoplasms (IPMNs) are included, a review of 99 studies of 9,249 patients with IPMNs who underwent surgical resection found that the incidence of either high-grade dysplasia or pancreatic cancer was 42% (ref. 3).

European and non-European experts performed systematic reviews and used GRADE methodology to answer relevant clinical questions on nine topics (biomarkers, radiology, endoscopy, intraductal papillary mucinous neoplasm (IPMN), mucinous cystic neoplasm (MCN), serous cystic neoplasm, rare cysts, (neo)adjuvant treatment, and pathology).

EUS-FNA-based cytology has good specificity but poor sensitivity in differentiating benign from malignant IPMNs. Newer techniques or markers are needed to improve diagnostic yield.

EUS examination of the whole pancreas plays an important role in the management of IPMNs as it allows the early detection of these small invasive carcinomas. IPMN-concomitant PDACs are quite often found at diagnosis and during follow-up.
