Kaido says it happens when your mind fully comes in sync with your powers so it really isn’t about lots of practice, it’s becoming one with the fruit and having that chemistry. It’s why some people can go a decade without awakening and other can awaken within two years.

Yes, the fruit is a fish model but the only forms that were shown are the dragon (Momo could use that without any effort or training) and the dragon-human hybrid (only Kaido yet as far as I know, who knows if Momo unlocks it down the line if he reappears)
