Not affiliated to death in any way. Even with its english dialogue it refers to the "zero world", which is pretty transparent. It appeared because Kuja refuted the whole cycle of life->death->reincarnation by trying to destroy its source, the crystal, because he couldn't see death as a part of the cycle for him.
