MRI T2WI, Sagittal Section. A large extra-axial, well-defined CSF intensity cystic lesion is identified in the midline in the sellar and suprasellar region. It measures 48 x 62 x 47 mm in AP transverse and cranio-caudal dimension. This lesion is causing a compressive effect on the third ventricle and bilateral lateral ventricles.

Isointense to Csf on all pulse sequences. Complete suppression of fluid signal on FLAIR. No restricted diffusion on Dw images. No septations or loculations. No calcification on T2*GRE.

There is a well-defined cyst lesion in the left lateral cerebellomedullary cistern, following the CSF signal in all the sequences, and not showing diffusion restriction. There is mild chronic impression over the adjacent temporal bone. The arachnoid cyst represents a benign condition, and the primary differential is made with an epidermoid cyst.
