MAPP Health Home Tracking System (HHTS) End-User Access There are several steps that are required in order for a staff person to access the MAPP HHTS. The first of these steps are: Obtain an HCS User Account (see your HCS Coordinator); Register for Multi–Factor Authentication (see your HCS Coordinator)

The Medicaid Analytics Performance Portal (MAPP) Health Home Tracking System (HHTS) is a performance management system that will provide tools to the Health Home network to support providing care management for the Health Home population.

The Medicaid Analytics Performance Portal (MAPP) Health Home Tracking System (HHTS) is accessed through the Health Commerce System (HCS). The HCS is a secure web portal managed by the NYSDOH. In order to access the MAPP HHTS, Health Homes, Managed Care Plans (MCP), and Care Management Agencies (CMA) must have a HCS organizational account.

The following procedure will assign MAPP roles to users in an organization. You must be an HCS Coordinator or you will not have access to the tool used in the following procedure.

MAPP users must have a HH type HCS account. If your organization has an existing HCS account but is not listed in HCS as a HH organization type, then you need to submit paperwork to request that your organization also be listed as a HH organization type in HCS.

To facilitate communication, the NYSDOH has defined two specific MAPP Roles in the HCS. These roles are the MAPP Single Point of Contact and the MAPP Gatekeeper.

Phase 1 of the MAPP Health Home Tracking System (HHTS) will provide users with the following functionality that is not in the current Health Home Tracking System: Care Management Agencies will have access to the MAPP HHTS. Actions within MAPP can be performed individually or in bulk through online screen entry or through file transfer.
