As of May 2024, there have been 170 documented crashes and 394 deaths involving the MU-2. [30] On 19 April 1993, an MU-2B-60 owned by the State of South Dakota, registration number N86SD, sustained extensive damage to the fuselage and left-hand wing and engine mounts after a propeller blade separation during cruise.

Reports state that the pilot had contacted the control tower at Joplin reporting icing on the aircraft’s wings and was turning back to land before all communication went silent.

Narrative:On his first Instrument Landing System (ILS) approach, the pilot initially flew through the localizer course. The pilot then reestablished the airplane on the final approach course, but the airplane’s altitude at the decision height was about 500 feet too high. He executed a missed approach and received radar vectors for another approach.

On 29 March 2016, a Mitsubishi MU-2 operated by Aero Teknic, a Canadian aircraft maintenance company, crashed on approach to Îles-de-la-Madeleine Airport, killing former Canadian politician Jean Lapierre, several members of his family, and the two pilots. Lapierre was flying to his father's funeral with his wife and three siblings. [1] .

There have been 330 fatalities from MU-2 crashes, alerting the FAA to begin safety evaluations. The aircraft has been concluded to be safe only when operated by properly trained pilots and when the aircraft is also adequately maintained.
