4AT: rapid assessment test for delirium detection. Designed by Alasdair MacLullich (University of Edinburgh), Tracy Ryan and Helen Cash (NHS Lothian). Used in clinical care by doctors, nurses, occupational therapists and other healthcare professional to detect and help diagnose delirium.

The 4A’s test (4AT): This is a short, four-item tool designed for use in clinical practice. The four items are alertness, cognition (a short test of orientation), attention (recitation of the months in backwards order), and the presence of acute change or fluctuating course.

2021年4月26日 · The 4AT (4 ‘A’s Test) is a brief tool for delirium detection, designed for use in clinical practice. It has been validated in 17 studies in over 3,700 patients. The test is currently used in specialist palliative care units, but has not been validated in this setting.

The 4AT is a screening instrument designed for rapid initial assessment of delirium and cognitive impairment. A score of 4 or more suggests delirium but is not diagnostic: more detailed assessment of mental status may be required to reach a diagnosis.

The 4AT is designed to balance sensitivity and specificity for delirium. It is also designed to yield a score even in situations where the patient is unable to speak or communicate meaningfully. Patients with severe dementia may have a level of impairment that means that they score 2 on both Items 2 and Item 3, giving a score of 4.

The 4 'A's test (Arousal, Attention, Abbreviated Mental Test - 4, Acute change) (4AT) is a screening tool designed for routine use. This project evaluated its usability, diagnostic accuracy and cost.

The 4AT is designed to be used as a delirium detection tool in general clinical settings, inpatient hospital settings outside of the Intensive Care Unit (ICU), or in the community. The 4AT is intended to be used by healthcare practitioners without the need for special training, and it takes around two minutes to complete. [ 4 ]

These guidelines were developed to assist clinicians in acute sites in NHS Lanarkshire in providing consistent and evidence based management in cases of suspected delirium in adults over the age of 18.

The 4 ‘A’s Test or 4AT is an assessment tool for delirium and cognitive impairment. Improved detection of delirium using the 4AT results in a conservatively estimated cost saving of GBP100 per patient. With 731,000 acute, emergency admissions for patients aged 75 and over per year in England alone, and 4AT used in 77% of NHS England acute hospital trusts, this can translate to NHS savings of GBP56,000,000 every year.
