We review characteristic CMR features in ischemic and non-ischemic cardiomyopathies (ICM and NICM), especially in terms of the location and distribution of late gadolinium enhancement (LGE). CMR in ICM shows segmental wall motion abnormalities or wall thinning in a particular coronary arterial territory, and the subendocardial or transmural LGE.

Recent studies suggest that traditional classification methods based on primary/mixed/acquired or genetic/non-genetic cannot meet the precise needs of contemporary clini-cal management. This article systematically describes the history of classifications of cardiomyopathy and presents etio-logical and genetic differences between cardiomyopathies.

While a minority of cardiomyopathies manifest with preserved systolic function, this article specifically reviews management of NICM manifesting with left ventricular (LV) systolic dysfunction. Many types of NICM are not treatable with specific therapies beyond those used for heart failure in general.

Delayed contrast-enhanced CMR can assist in evaluating functional decline, distinguishing between ischemic and non-ischemic causes of cardiomyopathy, and narrowing the differential diagnosis for NICM based on characteristic enhancement patterns.[4]
