2023年7月28日 · The posterior communicating artery originates from the posterior aspect of the C7 (communicating) segment of the internal carotid artery and extends posteromedially to anastomose with the ipsilateral posterior cerebral artery and forms part of the circle of Willis.

A left sided PCOM aneurysm is seen. Bilateral fetal PCOM can be seen also. otherwise the anatomy of circle of Willis is conventional. No evidence of subarachnoid bleeding (on FLAIR). No other lesion can be seen.

A fetal PCOM variant is defined as a PCOM artery, which has the same caliber as the P2 segment of the PCA and is associated with an atrophic P1 segment. Because fetal PCOM arteries are the primary supply to the PCA, care must be taken not to compromise flow to this artery during clipping or coiling of PCOM aneurysms.

Fetal PCOM are common incidental findings. Their importance is predominantly in the setting of vascular intervention (e.g. coiling of a PCOM aneurysm) or in the setting of stroke.
