2024年7月31日 · Chronic kidney disease (CKD) is characterized by the presence of kidney damage or an estimated glomerular filtration rate (eGFR) of less than 60 mL/min/1.73 m², persisting for 3 months or more. CKD involves a progressive loss of kidney function, often leading to the need for renal replacement therapy, such as dialysis or transplantation.

In 2002 the Kidney Disease Outcomes Quality Initiative (K/DOQI) of the National Kidney Foundation has published guidelines to define CKD and to classify stages in its progression. This classification system is based on the level of kidney function as estimated by glomerular filtration rate (GFR) regardless of the underlying pathology.

Consult nephrology urgently based on the clinical scenario. If eGFR <60 mL/min/1.73m2, repeat in 3 months to confirm diagnosis, or 1-2 weeks if concerned for acute kidney disease/injury (and no urgent evaluation indicated).

We describe the development and implementation of a chronic kidney disease clinical pathway (CKD-CP; www.ckdpathway.ca). The CKD-CP was developed and implemented based on the principles of the Knowledge-To-Action Cycle framework. We used a mixed methods approach to identify the usability and feasibility of the CKD-CP.
