The employer must complete an SH 900.2 Incident Report form, or an equivalent form, for each recordable injury or illness entered on the SH 900 Log.
Complaint forms are available at the Regional PEF Offices, PEF Training & Education, Health & Safety, or on the PEF website. The complaint must contain enough detail to allow the inspector to properly investigate. The form must be signed and dated.
