Anti-PL-12 is strongly associated with the presence of ILD, but less so with myositis and arthritis. Idiopathic ILD diagnosed as idiopathic pulmonary fibrosis may, in fact, be associated with anti-PL-12 and be a "forme fruste" of an underlying autoimmune disorder.

Anti-PL-12 is more closely associated with ILD than with other features of the antisynthetase syndrome, with the exception of Raynaud phenomenon. The myositis of anti-PL-12 tends to be milder or subclinical, suggesting that this antibody may indeed not be myositis specific, as previously designated.

Others include PL-7 and PL-12. Five additional antisynthetase antibodies have been identified so far, but there are much more rare and their implications are not well understood yet. Researchers are still identifying new autoantibodies of significance to myositis diseases.

Anti-Jo-1, anti-PL-7, anti-PL-12, anti-EJ, anti-KS, anti-OJ, anti-Ha, and anti-Zo antibodies target aminoacyl-tRNA synthetases and represent antisynthetase syndrome. Anti-Jo-1 is the most common autoantibody seen in antisynthetase syndrome and is present in an estimated 70-90% of patients with Myositis who have associated Interstitial Lung Disease.

We describe the case of a young lady with antisynthetase syndrome and diffuse glucocorticoid resistant non-specific interstitial pneumonia, who was salvaged and stabilized with a sequential combination therapy of cyclophosphamide and mycophenolate.

Patients and methods: Sera from patients with polymyositis and sera found to contain anticytoplasmic antibodies were screened for antibody to PL-12 by testing for inhibition of ARS enzymatic activity by serum, and by immunoprecipitation.
