The RSR Prime (rSR’) pattern represents a delay in activation within the basal part of the right ventricle (RV), which can be observed through distinct variations in QRS complex morphology. Several types of rSR’ patterns exist, such as incomplete or complete right bundle branch block (RBBB) patterns.

There should be a terminal R wave in lead V 1 (often called "R prime," and denoted by R, rR', rsR', rSR', or qR). There must be a prolonged S wave in leads I and V 6 (sometimes referred to as a "slurred" S wave).

In the present review, the conditions associated with an rSr’ pattern (the r’ usually is small but sometimes may be greater than “r”) are discussed and a diagnostic algorithm is proposed. In right bundle branch block (RBBB), the right ventricle depolarizes after the left ventricle, allowing its electrical vectors to become evident. These vectors manifest in the later part of the QRS complex as an R’ wave (pronounced “R prime”), which is directed anteriorly and rightward.

An electrocardiographic finding in which there are two R waves, which are two deflections above the baseline resulting from a single ventricular depolarization. The first upward deflection in the complex is the R wave. The S is the first downward deflection. A second upward deflection is called the R-prime wave.
