Many SDOH are out of your control. But with support from healthcare providers and community groups, you can gain access to resources your family needs to be as healthy as possible.
Many health systems are moving toward conducting SDOH screening to identify their patients’ nonmedical social needs and refer their patients to resources in the community with the overall goal of improving their patients’ and the overall community’s health and wellbeing.
