Bronchogenic cysts (BC) are a rare congenital pulmonary malformation which result from an embryological presence of the primitive foregut. The are usually asymptomatic, however at times may present with symptoms of cough, dyspnea, or post-obstructive pneumonias. BCs have a prevalence of 1:42,000–1:68,000.

Robotic-assisted thoracic surgery (RATS) offers precision and enhanced visualization, making it a safe and accurate approach for the removal of posterior mediastinal BCs.

The most common location is subcarinal, and the presence of a well-defined subcarinal mass in an asymptomatic patient suggests a bronchogenic cyst (Fig. 16-13). The right paratracheal region is the second most common location ( Fig. 16-14 ).
