Tension-free vaginal sling procedures: transobturator tape procedure (TOT) is a treatment for urodynamically proven stress incontinence in women. TOT is a minimally invasive procedure that involves the placement of a small piece of polypropylene mesh around the pubic bones underneath the urethra.

What is a TVT or TOT procedure? TVT (tension-free vaginal tape) and TOT (transobturator tape) are procedures that lift a woman’s sagging bladder or urethra into normal position. During surgery, a narrow band of tape is placed under the urethra. The tape supports the urethra and bladder like a hammock. Why do I need a TVT or TOT procedure?

TOT stands for TVT-Obturator, referring to its placement. It’s also referred to as TVT-O. The TVT-O passes through the obturator foramen and exits through the groin.

These bladder slings are now the gold standard in the surgical treatment of stress urinary incontinence. The TOT and TVT slings have almost wiped out every other surgical incontinence procedure in the last 50 years. These bladder sling procedures are quick 30-minute, outpatient procedures with a high success rate of approximately 90%.

What is a Transobturator Tape (TOT) surgery? It is a surgery to place a narrow ribbon of polypropylene mesh underneath the urethra. The urethra is the tube that allows urine to pass out of the body. The ribbon of mesh acts as a layer of support so that the urethra is closed off when there is downward pressure, like when you sneeze.

Surgical correction of SUI with TOT is the procedure of choice because of a shorter surgical time and a lower complication rate compared with TVT [8]. We focused on the morbidity and efficacy of the TOT procedure, including peri-, and post operative complications, and subjective results.

TOT is simple, minimally invasive and cost effective procedure for current surgical management for female stress urinary incontinence. When performed after proper training TOT sling procedure has 100% success rate (continence) and is associated with only minor postoperative complications which resolves within few days.

TOT fixation is a minimally invasive surgery for treatment of SUI. It has a high success rate as quoted in the literature and the total success rate of transobturator sling fixation in this study was also 93.2%.
