Trans-obturator (TOT) sling placement is performed at Urology Associates of Central California in our freestanding Ambulatory Surgery Center located on the same campus as your urologist’s office. During a trans-obturator sling procedure, your urologist will place the center of the sling beneath your urethra.

In transobturator tape (TOT) placement, through small incisions placed in the groins and in the vagina under the urethra, the mesh can be placed under the urethra in the correct position without having to pass needles blindly through the retropubic space, as in transvaginal tape (TVT).

Tension-free vaginal sling procedures: transobturator tape procedure (TOT) is a treatment for urodynamically proven stress incontinence in women. TOT is a minimally invasive procedure that involves the placement of a small piece of polypropylene mesh around the pubic bones underneath the urethra.

The TOT sling (transobturator tape sling) was invented in France in 2001 by Delorme and brought to the United States in 2002. The advantage of the TOT sling (in comparison to the TVT sling) is a decreased risk of bladder injury, bowel injury, and major blood vessel injury (because no retropubic needle passage is performed).

TRANSOBTURATOR TAPE (TOT) sling procedure is the gold standard for current surgical management of female stress urinary incontinence. TOT is simple, minimally invasive and cost effective procedure for current surgical management for female stress urinary incontinence.

TVT stands for tension-free vaginal tape. It’s also known as a synthetic mid-urethral tape or sling (MUS) because of the mid mark on the urethra where the device will be placed. Most TVTs are made of polypropylene, a polymer plastic also used in the top of Tic-Tac boxes and on the back of indoor-outdoor carpet.

3.8 Transobturator tape (TOT) procedure The transobturator tape was developed to avoid the blind passage of the needle in the retropubic space. Delorme first described this approach in 2001 [52].
