Vitreomacular traction (VMT) syndrome is a potentially visually significant disorder of the vitreoretinal interface characterized by an incomplete posterior vitreous detachment with the persistently adherent vitreous exerting tractional pull on the macula and resulting in morphologic alterations and consequent decline of visual function.

Focal VMT usually leads to macular hole formation, tractional cystoid macular edema and foveal retinal detachment, while broad VMT is associated with epiretinal membranes, diffuse retinal thickening and impaired foveal depression recovery.

Optical coherence tomography (OCT) is a highly sensitive test and is routinely used to diagnose and characterize ERM, VMT, and associated retinal changes. VMT features non-complete detachment of the posterior cortical hyaloid in the perifoveal area with a plate-like attachment on the center of the fovea and oblique anterior-posterior traction of the posterior cortical vitreous and may induce oval or tubular intraretinal cystic spaces [7].
