Tiny Tip: WOBBLER as an approach to ECGs for syncope
2021年8月10日 · The mnemonic, WOBBLER can be used as a memory aid for pertinent ECG findings for patients with syncope aside from obvious ischemic changes or dysrhythmias. Delta wave with PR shortening and QRS widening 2 . Obstruction of AV node (e.g. Mobitz type II, third degree heart block) or obstructive causes (pulmonary embolism, cardiac tamponade)
Spot the WOBBLER in syncope! | resusme
So we apply WOBBLER to screen the ECG of well-looking syncope patients without obvious ischaemia or dysrhythmia. Here goes… W is Wolff-Parkinson-White syndrome – look for a short PR interval or delta wave:
Wobble | Circulation: Arrhythmia and Electrophysiology
2015年8月1日 · Spontaneous changes in the measured intracardiac intervals during tachycardia (wobble) are useful in determining the mechanism of arrhythmia but require careful interpretation. The changes may be seen at induction of tachycardia, prior determination, or just after pacing maneuvers are deployed.
ECG Cases 5: Cardiac Syncope - Emergency Medicine Cases
2020年1月14日 · Take home points on ECG interpretation for cardiac syncope. Cardiac syncope often has high-risk features on history: palpitations or no prodrome (arrhythmogenic), exertional (HCM, aortic stenosis), or chest pain (ACS, PE, dissection)
Wobbly and Weak | Circulation: Heart Failure - AHA/ASA Journals
2018年8月6日 · ECG showed sinus rhythm, short PR interval, and baseline preexcitation with positive delta waves in leads I, II, and aVL and a QRS duration of 168 ms . He was discharged from the emergency department with a 30-day event monitor and instructions to have an echocardiogram before seeing cardiology.

Podcast 821: EKGs in Syncope - The Emergency Medical Minute
2022年10月17日 · This podcast discusses the mnemonic "WOBBLER" in identifying EKG findings after a patient has a syncopal episode
ECG Pointers: 7 Can’t-Miss ECG Patterns of High-Risk Syncope – …
2022年9月19日 · Here we introduce the mnemonic “ABCDE Left Right”, a memory aid to help EM providers quickly recall seven ECG patterns of high-risk syncope: AV block, Brugada pattern, QTc prolongation, delta waves (Wolff-Parkinson-White), epsilon waves (arrhythmogenic right ventricular cardiomyopathy), left ventricular hypertrophy, and right ventricular strain.

The nice thing about WOBBLER is that it uses the sequence that you follow when you look at an ECG, ie from left to right, or from P wave to T wave. The key is that this is for ECGs without obvious ischaemia or dysrhythmia. If you see something like this (STEMI): or this (VT): you don’t need WOBBLER, you need to be treating that patient.
