Failure Mode and Effects Analysis, or FMEA, is a systematic method used to identify potential failure points within a product, process, or system, assess their impact, and take action to mitigate or eliminate them.

Teams assign severity, occurrence, and likelihood of detection (SOD) values. Teams calculate a risk priority number (RPN), which is the product of SOD ratings. A FMEA team should include representation from design, test, reliability, materials, service, and manuf./process.

Potential failure mode affects safe vehicle operation and/or involves noncompliance with government regulation with warning. Or may endanger operator (machine or assembly) with warning. Loss of primary function (vehicle inoperable, does not affect safe vehicle operation). 100% of product may have to be scrapped. Line shutdown or stop ship.
