The requirements for submitting someone for a medal, whether it's an Achievement Medal or a Bronze Star, are largely the same but there are some differences. They all require the filling out of the DA Form 638 and for some medals, like the Achievement Medal and the Commendation Medal, that's all that's required.

2024年3月17日 · Currently filling out DA Form 638 for some of my guys for this past AT. Block 12A has various indication reasons, "ACH, SVC, PCS, ETS, RET". I wasn't sure what to put for them. I know what PCS, ETS, and RET are but what is ACH and SVC? I couldn't find out anything online. Go with achievement for an AT.

2016年1月11日 · You need to differentiate between the certificate and the 638. Both should have the Permanent Order number on them. The 638 has it on backside bottom (27b) from the orders authority. The certificate should have it typed on the bottom somewhere, usually under the signature block. Either one is a substantiating document.
