Amikacin (15 mg/kg/dose): Divide level by 3, then plot on graph Follow up monitoring: • An early trough (6-hours prior to dose) should be considered in patients demonstrating acute changes in renal function or
Aminoglycoside Calculator - ClinCalc.com
Advanced aminoglycoside pharmacokinetic calculator. Includes a variety of dosing strategies and calulation methods to determine an optimal gentamicin, tobramycin, or amikacin dose.
Evaluation of Amikacin Pharmacokinetics and Pharmacodynamics …
Amikacin has been one of the important antimicrobial agents against Gram-negative pathogens. However, there is discrepancy regarding the amikacin initial dosage, with some reports recently recommending ≥25 mg/kg and others the conventional dosage (15–20 mg/kg). Hence, we evaluated the optimal initial dosing regimen of amikacin.
Amikacin 15 -20 mg/kg INT-Q24H (round dose to the nearest 50mg increment) 15-20 mg/kg INT-Q 36 -48H (round dose to the nearest 50mg increment) For treatment of severe Gram-negative infections, dose recommended for gentamicin or tobramycin is 7 mg/kg and dose recommended for amikacin is 20mg/kg
Amikacin Pharmacokinetics/Pharmacodynamics in a Novel …
Amikacin concentration-effect response in test tubes. The graph shows impressive microbial kill after 3 days of incubation in test tubes. Surprisingly, the pattern of microbial kill, characterized by maxing out at 2 to 4 times the MIC, would be more consistent with time-dependent killing instead of concentration-dependent killing.
Population Pharmacokinetic Study of Amikacin Administered …
The graph of the predicted concentrations (more precisely, the individual predictions based on the population estimates of the values of the pharmacokinetic parameters for amikacin according to the covariates of each individual) versus observed concentrations is presented in Fig. Fig.2. 2. In this plot, the residuals (i.e., the difference ...

Amikacin - Infectious Diseases Management Program at UCSF
Obtain 6-14 hour random amikacin level after first dose to inform future dosing interval. Consider consulting pharmacy to assist with suggested dosing and monitoring. Use Barnes Jewish dosing nomogram. Do not use this dosing in CF patients
< 1 mg/L for gentamicin/tobramycin and < 4mg/L for amikacin (ideally undetectable though). • Appropriate dosing intervals include every 24, 36, or 48 hours
Therapeutic monitoring of amikacin and gentamicin in critically …
Comparing therapeutic serum concentrations of amikacin and gentamicin between critically and noncritically ill patients, we found that in critically ill patients gentamicin C max were significantly higher than in noncritically ill (P = 0.007) and amikacin C max were higher in critically ill patients but without statistical significance (P = 0. ...
