Cervical facet syndrome, also known as cervical facet joint pain, is a condition that occurs when the facet joints in the cervical spine become inflamed or damaged. The facet joints are small joints located between the neck vertebrae, which help to provide stability and facilitate movement.

Cervical degenerative facet disease, or arthrosis, causes four potential symptoms; neck pain, headaches, arm pain and instability pain.

The vertebrae in the C2-C5 motion segments are held together with ligaments and connected by paired facet joints running down the back of the spine. These joints are synovial joints that contain articulating cartilage to help facilitate smooth gliding movements between adjacent vertebrae.

The C2-C3 and C5-C6 joints are the most common cervical facet joints clinically implicated in neck pain. Interestingly however, C2-C3, C3-C4, and C4-C5 joints are the most likely to display findings of degeneration on diagnostic imaging such as X-ray, MRI, or CT.

As a facet joint ages, its cushioning cartilage dries out, gets small cracks and starts to wear down. Small tears and/or inflammation can also occur in the joint capsule and synovial membrane, which may affect the production of synovial fluid that helps keep the joint nourished and lubricated.
