After accepting the usual mandatory DoD Notice and Consent, the user is directed to Identity Authentication Services (iAS) page. Patient Safety Reporting (PSR) Event reporting is done through the JPSR, linked on your desktop, indicated by this icon. Choose DHA > CAC Login > “Report Event” on the top left. The PSM is notified upon reporting, at which point an investigation is initiated. You have a choice of anonymous reporting or not.
