This is not a title like 'Mr.' nor is a first name as western people understand. Although Md. is part of one's name, people never call anyone by this name.

The initials after the name just list educational or professional accomplishments: Master of Science, Bachelor of Arts, Diploma in Nursing, Registered Nurse Tutor. 2015年1月30日 · Since a person's name can be written with the surname first (e.g. formally or in a list), it is possible to have, for example, Freud, Sigmund MD. A medical doctor, or a doctor who's a lecturer in a university, has the prefix Dr, as in Dr John Black. It can have a full stop (Dr. John Black), but not a comma.
