The SSA-11-BK must be completed when the youth is approaching 16.5 or older but under 21. The youth must have a new medical and financial determination when they become 18. The process begins three months prior to the youth’s 18th birthday but is evaluated by SSA when the youth reaches 18.

An SSA-11 is required for converting a CDB who is capable to direct pay at age 18. However, if capability must be developed, you must obtain all needed documentation (see GN 00502.075 and GN 00504.105 ).

SSA requires individuals applying to be representative payees for OASDI, or SSI recipients to complete Form SSA-11-BK or, supply the same information to a field office technician through a personal interview.

The SSA-11-BK has been revised (2/2000) to include new SVB payee reporting responsibilities. Use the new SSA-11-BK when someone applies to be a payee for a title VIII beneficiary. Conduct a face-to-face interview whenever practicable.

Social Security's Representative Payment Program provides benefit payment management for our beneficiaries who are incapable of managing their Social Security or Supplemental Security Income (SSI) payments.
