During the spell there is hypercarbia and hypoxemia (which further increases pulmonary vascular resistance). The process compounds itself creating worsening right-to-left shunting, hyperpnea, right outflow tract obstruction and increased systemic venous return.

What are tet spells? Tet spells, also known as hypercyanotic episodes, are a more severe symptom of TOF. These sudden spells can occur after feeding, bathing, or crying. They are caused by a further decrease in blood flow to the lungs, making the baby more blue, and sometimes irritable and short of breath.

In severe cases the patient may present with tet spells. During tet spells the patients present with severe cyanosis, respiratory distress due metabolic acidosis and the severity of murmur is reduced7.
