The anterior bundle (also known as the ulnar collateral ligament [UCL]) is the primary restraint to valgus force at the medial elbow and is composed of two separate bands: the anterior band, which is tight in extension, and the posterior bad, which is tight in flexion.

Traditionally medial elbow injuries in overhead athletes have been treated with ulnar collateral ligament (UCL) reconstruction as first described by Dr. Frank Jobe. Recent literature has shown favorable outcomes in select patients treated with UCL repair.

What are the three bundles of the UCL? Anterior Band: Anteroinferior aspect of the medial epicondyle to the sublime tubercle (medial aspect of the coronoid process). Posterior Band: Fan shaped capsular thickening that runs from the medial epicondyle to the semilunar notch of the ulna. It is a stabilizer in full flexion.
