If the vaccine is eligible to be returned and you have not received a shipping label, please work with your RE to pull a Vaccine Loss Report (VLR). The RE will send the VLR and work with the vaccine ordering1 team to have a new return shipping label sent to the primary Vaccine Coordinator on file.

Click “Download VLR” on the pop-up and the file will open in a new tab as a PDF. Save and print the Vaccine Loss Report to obtain clinician signature. You can access older vaccine loss reports in VAOS for previous wastage or expiration that occurred.

The Vaccine Allocation and Ordering System (VAOS) is a platform built for the distribution and management of the COVID-19 vaccines, COVID-19 therapeutics products, and other vaccines. This guide gives a detailed walkthrough on accessing VAOS and instructions on how to order a COVID-19 therapeutics through VAOS.
